At Cedar Ridge Esthetic and Family Dentistry, we believe that everyone has a right to quality health and dental care. With the rising costs of health care in recent years, we understand the challenge this poses for many families. For this reason, we have developed a membership savings plan to make our quality care as affordable as possible.

Benefits to this plan include:

• No enrollment forms
• No waiting period – immediate eligibility
• No annual deductible
• No annual maximum
• No pre-authorization
• Cosmetic and Orthodontic treatments included
• Plan lasts full 12 months from start

Plan Details

Adult: $350

Couple: $600

Child (under 17): $200

What is Included:

• 2 Complete Exams
• 2 Routine Cleanings
• Complete Set of X-rays or Bitewings
• Other X-rays as needed
• Emergency Exams as needed (up to 2 per year)

Terms and Conditions

• All exam, x-rays, and cleanings must occur within 12 months of enrollment and cannot be rolled over if unused. There are no refunds for services unused.
• It is the patient’s responsibility to keep all appointments. Failure to call to reschedule and appointment without at least 24 hour advance notice will result in a $50 charge to your account.
• This plan does not apply to any other practice, even if referred by us.
• Not eligible to Workman’s Comp or auto accidents
• Membership cannot be combined with insurance or any other offer.
• Cannot pay for membership with Care Credit.
• Offer not applicable to home-care products such as electric toothbrushes or WaterPik.
